Medicaid provides health coverage to millions of Americans, including eligible low-income adults, children, pregnant women, elderly adults and people with disabilities. Medicaid is administered by states, according to federal requirements. The program is funded jointly by states and the federal government.
Patients please call the clinic to schedule an appointment before downloading new patient forms. Please provide these forms on day of your appointment, but do not mail them. The forms may be downloaded using the buttons below.
With a heavy heart, we are going to indefinitely close down the Walk-In clinics, effective immediately. We hope to find a resolution that allows us to open this back up at some point in the future. To be eligible for Florida Medicaid, you must be a resident of the state of Florida, a U.S. national, citizen, permanent resident, or legal alien, in need of health care/insurance assistance, whose financial situation would be characterized as low-income or very low income. You must also be one of the following:
- Pregnant, or be responsible for a child 18 years of age or younger
- Blind, or have a disability or a family member in your household with a disability
- Be 65 years of age or older
